Augmentin [Ovisaille
], an antibiotic formulation widely prescribed to both prevent and treat bacterial infections, is known to have a limited effectiveness against a wide range of pathogens includingStaphylococcus aureusandStaphylococcus epidermidis. It is an effective alternative to ampicillin for the treatment ofS. aureusinfections. The development of ampicillin has been one of the most effective methods for the treatment of bacterial infections, resulting in its widespread use in the management of many diseases such as sinusitis and otitis media (). However, the efficacy of augmentin against bacteria has not been thoroughly studied. This study aimed to evaluate the efficacy of augmentin in treating bacterial infections in pediatric patients receiving ampicillin for sinusitis and otitis media by using thein vitrobacterial growth assay andin vivoinhibitoryantibacterial drug concentration method. Additionally, this study investigated the antimicrobial activity of ampicillin againstepidermidisin a pediatric population (age 1 to 12 years) using theantibacterial concentration method. This study identified the potential to use ampicillin for the treatment of sinusitis and otitis media by evaluating the results ofMoreover, this study has also provided valuable information for the development of novel antibiotics based on theantibacterial concentration method for treating bacterial infections in pediatric patients.
The study was conducted at the Department of Pediatric Infectious Diseases, Faculty of Medicine, Gondar, Gondar, India, between February and July, 2020. Patients who were admitted to the pediatric department of our hospital from January to July of 2020 received ampicillin (Augmentin, 1.5 g/kg) and ampicillin/clavulanic acid (Augmentin, 2 g/kg) for the treatment of acute bacterial sinusitis and bacterial otitis media, respectively. Patients who received ampicillin/clavulanic acid alone or ampicillin/clavulanic acid combination therapy for both acute and chronic sinusitis were excluded from the study.
Adult patients who received ampicillin (Augmentin, 2 g/kg) or ampicillin/clavulanic acid (Augmentin, 2 g/kg) as the first-line treatment for acute sinusitis (AS) in pediatric patients ≤1 years of age, who were not receiving ampicillin for AS between January and March of 2020, or who had an active or chronic sinusitis within 1 year of admission, were included in the study. The criteria for AS were as follows: (i) acute AS; (ii) history of acute sinusitis within 1 year of admission; (iii) symptoms and signs of acute or chronic AS within the previous 2 weeks; and (iv) symptoms and signs of chronic AS within the previous 1 month.
The study was approved by the Ethical Committee of our hospital and was conducted in compliance with Good Clinical Practice Guidelines from the WHO and the Declaration of Helsinki. All patients gave informed consent and signed an informed consent form before enrollment in the study.
Thebacterial growth assay was carried out using the bacterial growth assay according to the manufacturer’s recommendations (Bio-Rad, Hercules, CA, USA). Briefly, 1 × 104 CFU/mL (approximately 1 g of) was inoculated into 96-well plates and incubated for 48 h at 37°C. The antibiotic effect was assessed by observing the bacterial growth inhibition of the treated bacteria by using the agar dilution method. The bacterial inoculum was adjusted to 1.4 g/mL of culture using a sterile, sterile, sterile 0.5% agar.
